1.  Print out the Order Form.

2.  Type or print clearly your shipping address.

3.  Please include your e-mail address for any needed quick correspondence.

4.  Indicate how many of each of the products you'd like to order.

5.  Calculate the total.

6.  Add $5.00 shipping and handling for the first product ordered, plus $4.00 for each additional unit.

7.  For orders outside the U.S.A., add $3.00.

8.  For orders to Maryland addresses, add 5% state sales tax.
